Health Record Access
Challenge: Although 90% of Ontario health providers use some form of electronic medical record (EMR) to digitally track their patient health data, these EMRs are not connected to Ontario's Electronic Health Record (EHR), which prevents health providers from seeing the "full picture" of a person's health.

How data was used: Funded by Ontario Health, and operated by Hamilton Health Sciences, ClinicalConnect, a secure web-based portal, was created to allow health providers real-time access to their patients' health data and provincial data repositories.
Result: When health providers can access their patient's complete health record, across care settings, in real-time through an electronic health record, they are able to make faster, more informed decisions, which lead to better health outcomes.
